The following paperwork must be submitted for each student:

  • Entry Form – To be completed on-line!
  • SSTFI Permission Form- Complete with ALL signatures
  • SRC Paperwork

 

SRC Paperwork - REQUIRED for ALL projects

  • Form 1 - Checklist for Adult Sponsor/Safety Assessment Form
  • Form 1A - Student Checklist
  • Research Plan
  • Form 1B Approval Form - 1 for EACH Student – DO NOT SUBMIT 1 per team!
  • Abstract

 

Additional SRC Paperwork

(Following forms are required ONLY if project meets certain criteria)

  • Form 1C - Regulated Research Institutional/Industrial Setting Form
  • Form 2 - Qualified Scientist
  • Form 3 - Risk Assessment
  • Form 4 - Human Subject & Informed Consent
  • Form 5A/B - Vertebrate Animals
  • Form 6A - Potentially Hazardous Biological Agents
  • Form 6B - Human & Vertebrate Animal Tissue
  • Form 7 - Continuation - MUST be completed for ANY continuation project no matter if the project has or has not been exhibited prior to the current year

NOTE: It is required that ALL projects involving humans, animals, potentially hazardous biological agents, and hazardous chemicals, activities or devices be approved before experimentation!

If you do NOT have a local SRC/IRB please contact the fair director before students begin experimentation.

It is important that students retain all original signed forms. Even though copies may have been sent with registration papers, students must bring original signed forms to the SSTFI in case an SRC interview is necessary.

NEVER send in original forms!!

Always send copies and teachers/parents should always maintain a copy on file.

These rules, guidelines, SRC procedure, and size limitations serve to promote the ethical principles of scientific research; insure the safety of participants, officials and guests; and to maintain the fair as a properly maintained and organized science fair according to international standards. A breech of these rules, guidelines, SRC procedure, and size limitation will DISQUALIFY the project from PARTICIPATION.
