Online ENTRY FORMS

  • Demographic student and school information,project information, table space and electricity needs

  • This information is used in building the judging databases, contact information, awardprogramprinting, media releases and for space and electrical requirements

  • NO lists are given out to any other group, organization or company – this information is confidential and used solely for the administration of the state science fair

  • Contact information is given to special award sponsors and scholarship sponsors to contact their award winners only

  • On-line entry is free of any charges (however, electricity fees must still be mailed with all otherpaperwork.

Manual ENTRY FORMS

  • These forms may be used locally to review the information that will be asked in the on-line entry.

  • It is discouraged to use these to actually enter aproject rather than utilizing the on-line system – to help discourage the use of these forms a fee will be chargedperproject.

  • DO NOT send in these forms with entries if you have entered on-line!

  • DO NOT use these forms if you find them confusing – they are meant for information and local on-line data entry only!

*If you will be attending a local or conference science fair after March 10th that is used by the teacher/school district to determine which students will be attending the state science fair you may submit papers after this date with

PRIOR APPROVAL FROM THE SSTFI Fair Director.

REQUIRED SRC FORMS

  • Forms dealing with safety of students, adult sponsors, and their research subjects (both human and animal)

  • In order for the SSTFI to affiliate with the INTEL International Science and Engineering Fair (ISEF) this paperwork must be submitted to and approved by the SSTFI Scientific Review Committee (SRC) before a student is allowed to compete at the SSTFI.

  • IN ORDER for the SSTFI to remain AFFILIATED with the Intel ISEF ALL STUDENTS regardless of age must complete required SRC forms.

  • The SSTFI must require these forms to remain affiliated but more importantly we are concerned for the safety of all students in our state engaged in science research.

  • This paperwork and all rules and guideline are provided by ISEF on their website: www.sciserv.org

INTEL ISEF SRC FORM ACCESS:

  • To access these forms use the www.sciserv.org website. http://www.sciserv.org/isef/teachers/forms_dates.asp
  • Intel ISEF website provides a rules wizard to help make the process of determining which forms are needed easier.
  • Forms provided on the Intel ISEF website have the ability to be:
    • downloaded
    • typed directly onto
    • easily printed in typed form
  • These forms may be printed blank and copied for classroom use.

These forms are required by ALL students (6th – 12th Grade).

SEMINAR ENTRY FORM

The seminar competition is an optional event at the SSTFI (students must be entered in the floor competition in order to submit papers for this event). Seminar competitors are selected through an initial research paper submission.

This event encourages students to fully utilize oral and written communication skills. Students are required to submit a written research paper. Students are encouraged to create a PowerPoint or other media presentation to present their research to a panel of judges.

Any student wishing to participate in the seminar competition at the SSTFI MUST COMPLETE the Seminar Entry Form during their online registration. The seminar entry form allows the Seminar Committee to quickly notify selected presenters, determine presentation needs and other logistics for the event.

IN ADDITION a complete project research paper must be sent with the seminar entry form to be eligible for competition. From the research papers received eight students in each class of each division (up to 72 total presenters) will be selected to give an oral presentation at the SSTFI.

This Seminar Entry form is for SSTFI use only and therefore is only found on the SSTFI website and in the SSTFI rule booklet.
